Application Fee
आवेदन हेतु प्रत्येक पद के लिए सामान्य कोटि / अन्य पिछड़ा वर्ग के आवेदकों को 400 /- रुपया एवं अनुसूचित / अनुसूचित जनजाति के आवेदकों के लिए 200/- रूपया का डिमांड ड्राफ्ट आवेदन पत्र के साथ संलग्न करना अनिवार्य होगा। ड्राफ्ट का भुगतान जिला स्वास्थ्य समिति, पूर्वी सिंहभूम के नाम से भुगतेय होगा।

- आवेदन को बंद लिफाफे में निबंधित डाक/ स्पीड पोस्ट के माध्यम से 16.08.2023 के अपराहन 05:00 बजे तक ही असैनिक शल्य चिकित सह मुख्य चिकित्सा पदाधिकारी का कार्यालय, पूर्वी सिंहभूम खासमहल, जमशेदपुर 831002 में स्वीकार किये जायेगे। निर्धारित समय के पश्चात प्राप्त आवेदन पर विचार नहीं किया जायेगा।
- आवेदन के साथ शैक्षणिक योग्यता/ तकनीकी योग्यता एवं अनुभव प्रमाण पत्र, आवासीय प्रमाण पत्र, जातीय प्रमाण पत्र, जन्म तिथि प्रमाण पत्र प्रवेशिका उत्तीर्ण प्रमाण पत्र, अंक प्रमाण पत्र की स्व- अभिप्रमाणित प्रति दो पासपोर्ट साईज का फोटो, बैंक ड्राफ्ट की मूल प्रति तथा स्वयं पत्राचार का लिखित लिफाफा में 32 रुपय का डाक टिकट संलग्न करना अनिवार्य होगा अन्यथा आवेदन अस्वीकृत कर दी जायेगी।
- किसी अन्य कार्यालय अथवा किसी अन्य माध्यम से आवेदन को स्वीकार नहीं किया जायेगा।
- लिफाफा के उपर स्पष्ट रूप से विज्ञापन संख्या,एवं पद का नाम, अंकित करना अनिवार्य होगा।
